Insurance Information
Physical Therapy 4 Kids is an out-of-network provider with most insurance companies. Payment is due at the time services are rendered, by cash, check, Venmo, debit or credit card (venmo and credit card payments are subject to a processing fee). We provide all of our patients with electronic claim submission on your behalf. This is not required; you are also welcome to submit the paper claims yourself if you prefer. For the New York State Health Insurance Plan (The Empire Plan/NYSHIP), pre-authorization is required and once received, we will bill the insurance company directly and the family only pays the copay at the time of the visit. For certain other insurance companies, we will make an attempt to get an In-for-Out Exception, a Single Case Agreement, GAP care coverage or a Network Deficiency exception whenever possible.
Payment Information
Payments are due at the time services are rendered. We accept cash, checks, Venmo, debit or credit cards.
Cancellation Policy
We request 24-hour notice when possible. We do understand that sudden illness and unexpected situations arise.
We reserve the right to charge $50 for a missed session with less than 24-hours notice.
