A Pediatric Physical Therapist should evaluate any child whose physical function could benefit from strengthening such as:

  • Atypical or poor posture
  • Asymmetrical body use (uses one arm or leg more than the other)
  • Awkward jumping or running patterns
  • Challenges in gross motor development (i.e., rolling, sitting, crawling, standing, walking)
  • Difficulty with balance and coordination
  • Excessive clumsiness,
floppiness or stiffness in muscle tone
  • Genetic disorders
  • Limited or excessive joint mobility
  • Plagiocephaly (flat head)
  • Toe walking
  • Torticollis (head tilted to one side)

We will talk with you about how Physical Therapy can benefit your child and set up a unique treatment plan to address your child’s and family’s needs. A successful outcome is our goal!

Call our office at 914-421-9392 to schedule an initial evaluation or fill out the form on this page and we will call you back. We will contact your doctor’s office for a prescription for Physical Therapy or you can ask the doctor for one once the referral is made. They can also fax the prescription to us at 914-946-8723.  It must have the child’s name and diagnosis on it. We work very closely with you, your child, your family members, and your doctor to maximize the results for your child!

Initial evaluations are one hour. Individual treatment sessions are 45 minutes. 

We schedule initial evaluations within 24-48 hours. Our primary goal is to evaluate, treat and graduate the child.  We work with your child and you so that you can achieve your goals in a timely manner.

Once your child is evaluated, our schedulers will set up recurring times for your child that work with your schedule.

Payment is due at the time services are rendered, by cash, check, Venmo, debit or credit card (venmo and credit card payments are subject to a processing fee). Physical Therapy 4 Kids is an out-of-network provider with most insurance companies. We provide all of our patients with electronic claim submission on your behalf. This is not required; you are also welcome to submit the paper claims yourself if you prefer. For the New York State Health Insurance Plan (The Empire Plan/NYSHIP), pre-authorization is required and once received, we will bill the insurance company directly and the family only pays the copay at the time of the visit. For certain other insurance companies, we will make an attempt to get an In-for-Out Exception, a Single Case Agreement, GAP care coverage or a Network Deficiency exception whenever possible.

Cancellation Policy: We request 24-hour notice when possible. We do understand that sudden illness and unexpected situations arise. We reserve the right to charge a $50 cancellation or no-show fee for the missed session with less than 24-hour notice.

Monday through Thursday – 8:00 am until 7:00 pm

Friday – 8:00 am until 2:00 pm

Saturdays – 8:00 am until 2:00 pm
